Disability Claim Forms:

American Fidelity, The Standard and UNUM claim forms are available with the Payroll/Benefits departments, www.americanfidelity.com www.thestandard.com and www.unum.com. Forms should be turned in to the Payroll office for completion. Upon completion, the payroll office will forward the original claim form directly to the appropriate insurance company and mail a copy to the employee’s home address.
