I hereby agree that if the transfer is allowed, it shall be subject to the following conditions as per BP5117 and AR5117:

  1. The student must maintain proper conduct and diligence in the school to which the transfer is made and his/her attendance must be regular. 
  2. The transfer is authorized only while the circumstances which warranted it prevail. 
  3. The parent assumes full responsibility for providing transportation. 
  4. Falsification of information on this form constitutes grounds for refusal or cancellation. 
  5. The transfer is subject to cancellation if the classroom becomes overcrowded. 
  6. No financial obligation shall be incurred by the district of residence. 

Por la presente acepto que si se permite la transferencia, estará sujeta a las siguientes condiciones según BP5117 y AR5117:

  1. El estudiante debe mantener una conducta adecuada y diligencia en la escuela a la que se realiza la transferencia y su asistencia debe ser regular.
  2. La transferencia se autoriza únicamente mientras concurran las circunstancias que la justificaron.
  3. El padre asume toda la responsabilidad de proporcionar transporte.
  4. La falsificación de información en este formulario constituye motivo de denegación o cancelación.
  5. La transferencia está sujeta a cancelación si el aula se llena de gente.
  6. El distrito de residencia no incurrirá en ninguna obligación financiera.
