Important Information Regarding Setting Up a American Fidelity Account:

1. Employees must make sure to open an account with American Fidelity at 800-365-9180 extension 0.
2. Completed forms must be received in the Benefits Office by the 15th of the month to be processed for the next payroll issuance.
3. American Fidelity deductions are a voluntary deduction.
4. Flex deductions are not directly deposited into an employee’s Flex account on payday.
5. The benefits department will need to audit before the funds are sent to the Flex account.
