New Hire Enrollment Qualifying Events, and COBRA

New Hire Enrollment:

Employee must contact our third party associates American Fidelity for consultation and to complete proper paperwork no later than 30 days from their date of hire at 800-365-9180 extension 0.  Take a look at the "Health Plan Policy Summaries" page as well to review our plans.

Notes on Member ID Cards: Member ID cards are only printed for medical insurance.  To identify your coverage at dental and vision offices, you will need to provide the name of your insurance carried as well as your social to the office.

Qualifying Event Enrollment:

Employee must contact the Benefits Department and complete proper paperwork no later than 30 days from the qualifying event.

COBRA Enrollment:

Employees will be contacted by our third party company SISC for Medical COBRA coverage after their exit date. For Vision or Dental COBRA coverage, employees will be contacted by the Benefits Department.
